Community Care Durham (CCD) is a registered charitable organization that has been serving Durham Region for decades. CCD provides a wide range of health, wellness, and support services designed to help adults aged 16+ and their caregivers maintain independence, improve quality of life, and remain safely in their homes. Their programs include Meals on Wheels, Accessible Transportation, Adult Day Programs, Assisted Living & Supportive Housing, Home at Last hospital transition support, Exercise & Falls Prevention, Foot Care Clinics, Friendly Visiting, Telephone Reassurance, In-Home Respite, and mental health support through WeConnect. CCD also offers volunteer opportunities, careers, and community engagement initiatives, making it a trusted partner in caring for the region’s most vulnerable populations.
